Should your child require adhoc or ongoing medical support, ie: daily medication, allergy action plans or support, please be sure to make contact with our dedicated First Aid Officer - Kathy Broadway.
Information can be shared with Kathy, by contacting the Administration Team on either wembley.ps@education.vic.gov.au or 03 9314 7054. Please be reassured that all information shared via the office will be kept confidential.
It is important that parents communicate any medical needs or diagnostic profiles with the school, so that we are best placed to support our students. If your child does require medication, a Medication Authority form must be completed and lodged with the school - please see below attachment. This is also the form that should be used for adhoc medication during camps etc.
It is essential that all students with the following profiles, have an authorised ASCIA management plan from their medical team:
- Asthma Management
- Anaphylaxis Management
- Allergy Management
- Epilepsy Management
